From the first day you join our office, we will cater to your dental needs.
As part of your first visit with us, you fill out a comprehensive medical and dental history form. Those forms are important they will help us determine the course of your treatment. We have to know about allergies, sensitivity to anesthetics, long-term medications, etc.
For your convenience you can download and complete them at your leisure before you come to your appointment.

Your First Visit
On your first visit we will conduct a comprehensive examination and periodontal evaluation. Necessary X-rays will be taken in order to fully examine you, as it gives us a picture of the tooth structure above below the gum line, as well as enables us to evaluate the bone structure surrounding your teeth.
After gathering all of the necessary information, your treatment will be reviewed with you. This allows us to develop a personalized plan for your oral care. We will work with you so that your oral care plan will be convenient for your life style and busy schedule.
We respect your time and make every effort to remain on schedule. If you are unable to keep an appointment, kindly notify our office immediately.
We would appreciate a minimum of 24 hours’ notice if an appointment must be changed.
Please make sure to bring :
- Your insurance card; if an insurance policy should be considered for this visit
- A form of picture identification.
- List of any medication you might be taking

Insurance Verification and Pre-Approval
All Insurances must be verified prior to your dental appointment.
Please provided your insurance information at the time to make your appointment.
We accept most Preferred Provider Organization (PPO), Health Maintenance Organization (HMO or DHMO), Dental Plans and Fee-Schedule based insurances.
Consult your insurance provider for details concerning which procedures are covered by your policy.
Financial Agreements
We believe there is no better investment than in your oral health.
We work very hard to control the cost of dental care. Prior to any work being performed, we will sit down together and go over estimates of fees specific to your treatment plan.
Insurance helps with the cost of your dental care. Each policy is different. If you have insurance, we will file to for you. At each visit you will be expected to pay only the estimated percentage of your total fee the portion not covered by insurance. When the payment from the insurance company is received and applied to your account, any balance due will be billed to you, over payments refunded to you.
For patients without insurance, we ask that you pay for services on the day they are completed.
